HealthHive is the virtual home base for patients and the people that care about them. HealthHive offers a patient Hive, which is owned and controlled by the individual (let’s call her Martha). The Hive centralizes all of Martha’s information, regardless of its source, and allows everyone interested in Martha’s well-being access to her existing clinical data (i.e., from her range of physicians, hospitals, and other providers), daily activities, preferences, health goals, and so much more. Further, the Hive provides a means of sharing information and communicating amongst all of these parties. The Hive Portal provides an organizational view of the patient Hives, enabling the management of patient populations. In particular, the Hive Portal helps in the management of care transitions and care management outside of the clinical setting.
